Butters finds out what his father has been up to late at night and when his mother hears, she overreacts by trying to kill her son and then herself back at home. After returning home and forgiving her husband, she realizes what she has done to her son (she doesn’t know she failed) they spin a story to the media to cover their tracks. Meanwhile, Butters obsesses with returning home so that he can join his parents for an anniversary dinner at Bennigan’s.

Kenpachi Zaraki, captain of the 11th squad, is running through the Soul Society with Orihime and Yachiru on his back. After getting lost again in the streets of Soul Society, Kenpachi runs into Captain Tousen and Captain Komamura and their vice captains, Shuuhei Hisagi and Tetsuzaemon Iba. Tousen asks Kenpachi why he is helping the ryoka and instead of answering him, Kenpachi challenges them to a fight. Meanwhile back in the cavern, Renji finishes his training and asks Yoruichi if Ichigo can really achieve Bankai in time.

As House and his team are working on the diagnosis of a man with a giant, swollen tongue, the husband of a former patient walks into House’s office and shoots him. House continues to treat his patient from his ICU hospital bed with the shooter, who has been shot by hospital security and hand-cuffed to his bed, as his roommate. When the after-effects of the shooting begin to impact House, he starts to question his own ability to diagnose properly. As his patient’s body deteriorates, House struggles through his self-doubt and must trust his team to find a way to solve the case.

The Sanbi creates a thick mist that can create illusions, forcing everyone to leave the area. Elsewhere, Guren awakens by Yukimaru's side and discovers a dangerous side-effect to his abilities. Not willing to let Orochimaru get a hold on the Sanbi, Tsunade sends a back-up unit to detain it so Konoha may protect it.
The Sanbi rises from the lake and Guren attempts to capture it, but the Bijuu refuses to be captured. Kakashi, Naruto, Kiba, Shino and Yamato follow the bat to their enemy's location and Kakashi has Naruto investigate the lake while he and the others distract Guren's group. Meanwhile, Akatsuki decides to make a move.

Hidan falls into Shikamaru's trap, but now that he has Hidan cornered, can he find a way to defeat Hidan's immortality? Elsewhere, things are looking grim for Kakashi, Choji, and Ino as they continue fighting Kakuzu, but a surprise back-up unit arrives on the scene to provide some much needed assistance.
Shikamaru continues his fight with Hidan. Hidan manages to injure Shikamaru and curses him the same way he did Asuma, but when Shikamaru survives Hidan's attack, Hidan realizes something isn't quite right.
As Kakashi and the others continue to struggle against the Akatsuki duo, Kakuzu reveals that he has not one, but five hearts! In order to kill him, one must take out all five of his hearts. Meanwhile, Shikamaru decides to challenge Hidan alone, and forces him away from his partner.
When Kakashi strikes Kakuzu through the heart, the battle seems to be over. However, when Kakuzu not only survives the attack, but also manages to escape from Shikamaru's trap, it soon becomes clear that the fight is only just beginning.
All of Konoha mourns the tragic loss of Asuma Sarutobi. Knowing that Hidan and Kakuzu are bound to return, Shikamaru, Choji, and Ino prepare to head off to take revenge upon the duo while Kakashi takes temporary leadership of the team.
